Subscription

Our subscription plan costs $75.00 per household. It will cover out-of-pocket expenses except for any amounts due as part of an insurance deductible or co-pay. Most insurance plans, including Medicare, do not cover 100% of the cost for ambulance service. Participation in this program ensures that you will not be responsible for high out-of-pocket costs associated with 9-1-1 emergency ambulance service provided by Yardley Makefield Emergency Unit.

Our additional benefit offered with the subscription is four medical assists covered at no cost to you!

That means if you need our assistance but do not require transport to the hospital, we will come to your home and do what is necessary to aid and comfort you at no additional cost. This is our way of giving back to the community we serve.

How to Submit Your Form

Once completed, submit the form one of two ways:
